Your First Time
As a new patient at Kneading Touch, we will have you fill out certain forms before your first massage. There are two ways you can address this, either download the forms available here and fill them out before you arrive, bringing them with you to your first appointment. You may chose to do so in our waiting area before your first appointment, if you choose to fill them out onsite in person, please allow 10 to 15 minutes before your scheduled appointment time to complete our forms.
